We offer specialized evidence-based services for patients experiencing a variety of psychological difficulties, including Mood Disorders, Anxiety/Obsessive-Compulsive Disorders, Psychosis, and Addiction. We offer specialty programs in Women’s Behavioral Health, Trauma, and Neurocognitive Disorders. The Social Worker serves as a critical part of our clinical team, working within the Interdisciplinary Treatment Team to social work services. Patients and their family will experience the benefits of patient-focused care through the integrated skills of all treatment team members.
** POSITION SUMMARY
** This clinical support position serves one of the Department of Psychiatry & Behavioral Health’s
** Psychosis Program.
** The Social Worker facilitates an efficient, effective, and compassionate transition through levels of care, ensuring outpatients are linked to needed services. The Social Worker monitors the patient care process and collects data to evaluate and improve timeliness and appropriateness of care. The Social Worker collaborates with physicians, psychologists, and other health care workers to provide comprehensive case management related to treatment, follow-up evaluations, and communication of care plan.
The Social Worker provides services to support the mental and physical health of patients in collaboration with other members of the care team. The Social Worker maintains an awareness of state and national health care trends and proactively responds with initiatives to enhance provision of care and ensure efficient resource utilization.
Clinical, leadership, and education position, including, but not limited to:
* Coordinate the clinical activities of outpatient team members to achieve the most effective, efficient and evidence-based service delivery to patients & families.
* Monitor and assess practice patterns to ensure a high level of clinical expertise and positive outcomes for patients and families. Reports outcomes to management.
* Provide triage and diagnostic evaluation for patients with mental health complaints.
* Provide psychotherapy to individuals with mental & behavioral disorders to facilitate return to previous or improved functioning. Services provided include individual psychotherapy, group psychotherapy, and/or couple/family therapy.
* Formulate individualized treatment plans to assist patients to determine and meet treatment goals.
* Other duties include advocating within local community if needed; work as part of clinical team; maintain third-party payer relationships; and complete all necessary paperwork (e.g., disability, FMLA).
** MINIMUM REQUIRED QUALIFICATIONS
** Master's Degree in Social Work or Counseling. Independent License. LISW or LPCC. 2 years of relevant experience required. 4-6 years of relevant experience preferred.
