Absolute Care is seeking a Community Health Worker who will connect members with resources to address social needs and barriers to care. You will educate new members about Absolute Care and guide enrollment in services, working across the care management and engagement teams in a matrixed environment.
The role requires CHW certification, CPR/BLS, and a valid driver’s license. Experience with vulnerable populations and strong Microsoft Office skills are a plus. Travel locally is expected.
