Essential Functions & Responsibilities

• Provider responsible for rounding and managing clinical care of residents at assigned SNF.
• Provider responsible for rounding daily, prescribing medications and treatments for residents.
• Provider will provide high quality, patient-centered, comprehensive primary care services to patients residing in SNFs.
• Provider responsible for timely documentation of all patient care encounters.
• Provider available for phone/secure messaging consultations with facility nursing staff.
• Provider documentation completed in Theoria’s proprietary EHR system, Chart Easy™.
• Point Click Care , Matrix Care, and American Health Tech are commonly used facility EHR systems.
• Provider expected to be on-site and on time for scheduled shifts.
• Timely attendance imperative.
• Reliable transportation required.

• Rounding time 7AM-1PM local time; provider must be available to nursing staff.
• Shifts begin around 7AM local time with flexibility.
• Daily Monday-Friday shifts.
• Charting/documentation can be completed off-site.
• This is an in-person position.
