Total Extended Care Services, a growing Physical Medicine and Rehab group, is looking for a dedicated Nurse Practitioner/Physician Assistant to join our team. In this role, you will partner with our PM&R physician in the sub-acute rehab setting. The position is ideal for those passionate about improving patient outcomes and working within an interdisciplinary, specialized team.

Collaborate with Board-Certified Physical Medicine and Rehabilitation Physicians along with other Nurse Practitioners and Physician Assistants trained in PM&R - Flexible Schedule

Help improve patient functionality and satisfaction in senior living communities - On-Site Pain Management

Work with medical providers, therapists, social workers, nurses, and families to ensure optimal patient care

In-person at skilled nursing facilities

Flexible options – Full-time and Part-time positions available

Partner with the facility's therapy department to provide the best possible rehab care. Communicate and collaborate with the patient’s primary care team to ensure the best outcomes. Timely charting/documentation is required but can be done from home.
