How You'll Make a DifferenceAs a Registered Nurse Case Manager (RNCM) you will make a difference in the lives of our patients. Our nurses give our patients the greatest gift – the ability to spend enhanced quality time with their loved ones in their preferred environment.
As a RNCM you will provide one‑on‑one patient care, work at the top of your license, and leverage your leadership skills to coordinate care. You will provide home‑based nursing care for patients directed by an attending physician, collaborate with an interdisciplinary team, and interact with patients’ families.

Assume ultimate accountability and leadership for patient assessment and plan of care. Case manage and provide clinical care under physician direction, educate patients and families, and ensure patient safety. Work autonomously and manage time efficiently.
Major Areas of Responsibility- Plan of Care:
Observe and monitor patient conditions and perform OASIS assessments to develop and adjust individualized care plans. - Minimize Patient Risk:
Administer medication, prioritize high‑risk visits, and reduce re‑hospitalizations. - Supervision:
Oversee care provided by nurse aides and LPNs. - Continuity of Care:
Manage multidisciplinary care, promote continuity with admissions, transfers, and discharges. - Family Educator/Advocate:
Educate patient and family using teach‑back methods. - Policies:
Complete clinical documentation following agency protocol and Medicare/Federal guidelines. - Rules and Regulations:
Follow agency policies and communicate schedule changes to supervisors. - Operations:
Attend in‑service training and mandatory agency meetings.
- Compassionate communicator with a positive attitude.
- Patiently work with patients, families, physicians, and coworkers.
- Attention to detail and observant following directions.
- Registered Nurse with a current license in the state of employment.
- Minimum one‑year experience as an RN in an acute care setting; home care experience preferred.
- Valid driver’s license and auto insurance in your name.
- Capable of all physical demands.
