PRN Primary Care Nurse Practitioner – Care Med Health Solutions

Care Med Health Solutions is a facility‑based team of medical providers who deliver medical services to patients living in long‑term care facilities. We strive to provide quality care to residents while collaborating with the facility to deliver the best possible outcomes. Working as part of an interdisciplinary care team, you will have the opportunity to positively impact the lives and well‑being of the elderly population.

• Provide primary and preventive care for long‑term care patients residing in skilled nursing facilities.
• Manage residents with acute changes in condition.
• Perform advanced care planning and chronic care management.
• Prescribe appropriate medications, including controlled substances, in accordance with OH law.
• Order, review, and follow‑up on treatments and diagnostic testing.
• Communicate with nursing home staff regularly to discuss changes in condition and care plans.
• Collaborate with the primary care physician.

• Current, unrestricted NP license in the state of Ohio.
• Board certification.
• DEA licensure.
• Clinical background in adult, family, or geriatric specialties.
• Experience working in a long‑term care setting.
• Ability to work with an electronic medical record (EMR).
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team to coordinate patient care.
