Job Description Summary

Schedule:

12 hour shifts, 3x a week from 7am-7:30pm, with 5 weekend shifts in a 6 week scheduling period and Works 2 holidays per year on a 3 year rotation

Join the Ohio Health Hospice team as a Hospice Registered Nurse at Kobacker House and make a meaningful impact by supporting patients and families through compassionate end‑of‑life care. In this role, you’ll coordinate individualized care plans, conduct patient assessments, and provide education and guidance that helps patients remain comfortable. Your clinical expertise and ability to build trusting relationships will play a vital part in supporting symptom management, safety, and dignity for those we serve.

As a key member of an interdisciplinary care team, you will collaborate with physicians, social workers, chaplains, aides, and navigators to ensure seamless communication and holistic support for patients and their loved ones. This role is ideal for nurses who value critical thinking, strong organization skills, and meaningful patient care. Hospice experience is helpful, but strong candidates may also come from backgrounds such as home health, oncology, palliative care, or other roles requiring high‑touch patient coordination.

Responsibilities And Duties

90% - Provides direct patient care in accordance with Home Care policies and specified procedures and evaluation of patient/caregiver response to institutions. Demonstrates and provides comprehensive documentation of services rendered. Proficiency in the performance of procedures and equipment. Assesses potential safety issues, both for patient and Home Care staff. Gathers baseline data and records assessment in patients Electronic Medical Records (EMR);

identifies problems, develops patient specific goals, and develops a Plan of Care (POC) with collaboration of the intradisciplinary care team based on physician orders and patient collaboration and establishes realistic patient outcomes of care. Continuously assesses patients and changing needs. Notifies physician with any change. Provides coordination of services with the intradisciplinary team other Home Care departments, subsidiaries, and community resources.

Completes documentation in a timely manner and within policy guidelines completes addendum orders the day the order is given. Updates POC when order is given. Documents all phone calls to physicians.

5% - Engagement and taking an active role in development and the betterment of the associate and the organization

5% - Completion of duties as assigned that pertain to compliance, quality, culture, and excellence of the organization

Minimum Qualifications

Associate's Degree (Required) DL - Driver's License - Department of Motor Vehicles, RN - Registered Nurse - Ohio Board of Nursing

Additional

Job Description Specialized Knowledge
  • Knowledge of home care nursing, home intravenous therapy, Medicare regulations, JCAHO standards, home care reimbursement

  • Basic personal computer skills

  • Knowledge in assessment, planning, implementation, and evaluation of nursing care
