Dulla Care Home Healthcare is creating the most advanced and complete in-home care model in the world. As a key contributor toward that goal, the RN will serve a pivotal role in bringing individualized care to clients’ homes as an alternative to hospitalization. The RN will bring advanced clinical, communication, and leadership skills to our innovative team.

Responsibilities
  • Provides direct and indirect nursing care to clients in home care settings, including admission framework.
  • Supervises and oversees home care staff; appropriates discrepancies as warranted.
  • Performs lab draws, IV insertion, and medication administration.
  • Engages clients in their healthcare journey and provides meaningful tailored education.
  • Establishes meaningful connections and trusting relationships with each client.
  • Appropriate charting, noting in OASIS and/or medical records.
  • Implements multidisciplinary plan of care for clients in the Dulla Care Home Healthcare program services.
  • Works closely with the leadership team to support and promote the care redesign of 24/7 comprehensive care delivered in the home.
  • Oversees the planning, scheduling, and implementation of day‑to‑day clinical activities for clients.
  • Ensures appropriate DME and services are ordered and initiated in a timely manner.
  • Acts as a central point of contact for all navigated clients, including interaction with medical, clinical and care staff.
  • Collaborates with medical providers, client care staff and skilled services in planning and implementing client and education plans.
  • Participates in client education and engagement; ensures educational goals are on track and actively assesses client understanding.
  • Monitors clients daily for compliance with the plan of care and tracks order completion and follow‑up, engaging with the client throughout the episode of care.
  • Assists clients and care partners with referrals to community resources and care coordination that can assist with social determinants of health as needed.
  • Monitors client’s progress to goals in the post‑30‑day period.
  • Participates in the Dulla Care Home Healthcare’s materials (education materials, assessment plans, interdisciplinary plan of care, etc.).
  • Completes daily EVV check‑in and check‑out.
What You Need
  • BSN preferred.
  • Certification/licensure in the State of Ohio as a Licensed Registered Nurse or licensure from a state participating in the multistate nursing compact with Ohio.
  • Minimum of 7 years acute care or home care nursing experience.
  • BLS and ACLS certified.

Physical Requirements
  • Lift & carry equipment/supplies weighing up to 50 pounds over long distances.
  • Push or pull objects weighing up to 50 pounds.
  • Walk long distances and up/down multiple floors of stairs.
  • Twist, bend, stoop/squat frequently throughout the day.
  • Reach and grasp with arms and hands, including overhead and below the waist.
  • Move within confined spaces.
  • Maintain physical tolerance and balance in multiple positions.
  • Stand (at client's side during treatment, etc.).
  • Sustain repetitive movement (CPR compressions, bagging, etc.).
