Summary:

** Ensures care continuity for patients and families and collaborates in the development, implementation, and evaluation of all program components.

* Essential Functions:

*** Develops, implements, and enhances special projects promoting inpatient/outpatient continuity of care and parent engagement.
* Ensures the provision of high quality, individualized, comprehensive, cost effective patient across the continuum.
* Initiates, tracks, analyzes, and manages all program quality improvement and research projects, program goals and objectives.
* Coordinates functions with interdisciplinary medical care, liaison functions with patient, family, medical staff, and referring physicians.
* Develops educational materials and programs for patients, nursing and ancillary staff, and physicians.
* Develops clinical protocols, policies, and procedures including community education and public relations.  
** Education Requirement:
*** Bachelor's Degree, required.
* Masters Degree in Nursing, preferred.  
** Licensure Requirement:
** Registered nurse in the State of Ohio, required.

* Certifications:

* Skills:

*** Effective customer service skills.
* Good multiplatform communication skills (oral, written, presentation).
* Ability to assist in conflict resolution.
* Problem-solving and interpersonal skills required for interaction with contacts required.
