Case Manager - Registered Nurse - South Region

The Case Manager - Registered Nurse position at CVS Health involves a collaborative process to assess, plan, and coordinate care for individuals and families, focusing on comprehensive health needs. • Use a collaborative process for assessment, planning, and care coordination.
• Act as a liaison with members, families, employers, providers, and insurance companies.
• Implement and coordinate case management activities for catastrophic cases and chronically ill members.
• Interact with members telephonically or in person, including home visits.
• Assess and analyze medical and vocational status of members.
• Develop plans of care to optimize wellness and medical outcomes.
• Communicate with stakeholders regarding member care.
• Prepare documentation of case work activities.
• Consult with internal multidisciplinary teams to maximize health outcomes.
• Monitor member progress toward desired outcomes.

Education:

• Associate's Degree or Nursing Diploma (REQUIRED)
• Bachelor's Degree (PREFERRED)

Experience:

• 3+ years clinical practical experience (diabetes, CHF, CKD, post-acute care, hospice, palliative care, cardiac) with Medicare members
• 2+ years case management, discharge planning, and/or home health care coordination experience

Licenses &

Certifications:

• Active and unrestricted Compact Registered Nurse (RN) Licensure in OH or in state of residence
