Pay: $25.27–$31.58 per hour

SummaryThe Referral Coordinator works under the supervision of the Referral Coordinator Supervisor and the Assistant Director of Ambulatory Operations to support Stony Brook Medicine’s patient‑access goals. The role ensures timely, accurate, and patient‑centered processing of referrals across all specialties and providers, acting as the liaison among referring providers, specialists, patients, and support staff. The Coordinator promotes continuity of care, reduces access barriers, and enhances the overall patient experience.
Job Duties & Essential Functions- Act as the main liaison among physicians, patients, and specialty departments.
- Monitor daily referral queues to ensure processing within established time frames and quality standards.
- Verify referral requirements based on insurance plans, provider preferences, and internal policies.
- Coordinate incoming and outgoing referrals, ensuring all necessary clinical documentation is obtained and transmitted accurately.
- Communicate effectively with patients regarding referral status, appointment scheduling, and insurance authorization.
- Track referral completion and follow‑up to maintain continuity of care and documentation in the electronic health record (EHR).
- Ensure compliance with payer requirements for pre‑authorizations and network referrals.
- Maintain strong working relationships with physicians, office staff, and external specialty offices.
- Participate in interdepartmental meetings to improve communication between referral sources.
- High‑school diploma or equivalent.
- Associate or bachelor’s degree in healthcare administration preferred; otherwise 5 years of medical office operations or referral coordination experience.
- Minimum 3 years of healthcare referral experience.
- Highly developed organizational, problem‑solving, interpersonal, and oral and written communication skills; proficiency in Word, Excel, and PowerPoint.
- Demonstrated commitment to patient confidentiality and quality service delivery.
- Bachelor’s degree.
- Experience with prior authorization requirements and referral processes.
- Familiarity with the Cerner electronic medical record and Cerner Practice Management module.
The role is largely sedentary and requires the employee to remain stationary for the majority of the day. The employee must communicate regularly with patients, staff, and medical providers. Additional physical demands, if any, will be outlined by management.
