Gurwin Jewish Nursing & Rehabilitation Center is a renowned healthcare provider located on Long Island, NY which offers a full continuum of services, including short‑term rehabilitation, long‑term skilled nursing care, home care programs, an adult day health program, ventilator‑dependent care, palliative and hospice care, a memory care unit and on‑site dialysis. Fay J. Lindner assisted living community and the Fountaingate Gardens independent living community on the Commack campus.

We are currently seeking per diem Nursing Care Coordinators (NCCs) for the evening shift.

Shift: 3:00 pm – 11:15 pm, with a minimum of one weekend per month (Saturday and Sunday) and additional time as needed. Must be available for a minimum of one week of orientation from 8:00 am – 4:15 pm.

• The Nursing Care Coordinator coordinates direct nursing services to patients in collaboration with unit staff and nurses.
• The NCC assumes both managerial and clinical roles, ensuring adherence to institutional policies and practices through collaboration with other disciplines.
• The NCC maintains nursing staffing levels and manages patient care requirements throughout the facility.

• Graduate of an accredited School of Nursing.
• Licensed to practice as a Registered Nurse in New York.
• Progressive experience in Nursing Administration.
• Strong leadership and administrative ability.
• Bachelor’s degree in nursing preferred.
