Licensed Practical Nurse LPN – Sign on Bonus $10,000 (FT) / $5,000 (PT)
Join Gurwin Healthcare System as a Licensed Practical Nurse (LPN). The role offers a $10,000 sign‑on bonus for full‑time hires and $5,000 for part‑time hires.
PayBase pay: $34.36/hr – $37.80/hr (full‑time). Rate increases to $36.19/hr – $39.81/hr after six months of employment.
Location & Shift ScheduleCommack, NY – Gurwin Jewish Nursing & Rehabilitation Center (460‑bed). Full‑time LPNs work evening and night shifts: 3:00 pm–11:15 pm and 11:00 pm–7:15 am, scheduled every other weekend. Part‑time LPNs work all shifts: 7:00 am–3:15 pm, 3:00 pm–11:15 pm, and 11:00 pm–7:15 am, scheduled every other weekend.
Requirements- Graduate of an accredited professional nursing program with a current license to practice in New York State.
- Interest in the special needs of geriatric patients and ability to integrate geriatric nursing principles into practice.
- Effective interpersonal relations with patients, staff, and families.
- Effective communication in English, both oral and written.
- Education in all areas of LTC/Subacute to ensure competency on all facility units.
- Participates in nursing care of patients within the scope of practical nursing, under the supervision of an RN.
- Forms and implements patient care plans to meet specialized needs of geriatric patients under RN supervision.
- Administers patient care as assigned by an RN within departmental guidelines; may assume team leader, primary nurse, or charge roles under RN direction.
- Directs and supervises non‑professional staff as assigned.
- Observes, records, and reports patient data per departmental policy.
- Participates in unit nursing and interdisciplinary team activities.
- Administers medications and performs nursing procedures per departmental policy.
- Attends institutional in‑service programs and participates in departmental committees as assigned.
- Performs related and miscellaneous duties as directed; may be required to perform unrelated duties in emergencies.
