Set on a 36-acre campus in Commack, NY, the Gurwin Healthcare System includes the 460-bed Gurwin Jewish Nursing & Rehabilitation Center, providing short-term rehabilitation and long-term skilled nursing care, memory care, respiratory and ventilator care, palliative and hospice care, social and medical adult day programs, and on-site dialysis and infusion therapy.
Sharing the campus is Gurwin’s Assisted Living community (Gurwin Jewish ~ Fay J. Lindner Residences) which has 210 beds, our independent living community (Fountaingate Gardens) and Gurwin Certified and Licensed Home Care Services Agencies.
Our innovative facility and growing services offer opportunities to advance your career. We are currently seeking a full-time Assistant Director of Resident Services who will embody the passion and commitment required to deliver the ultimate care for our residents.
About this role:
$10,000 Sign on Bonus
You will be scheduled to work Sunday - Thursday, 8:30a.m.
- 5:00p.m., along with on-call responsibilities.
Requirements include:
Current valid New York State License
Minimum 2 years LPN experience, preferably with the geriatric population
Minimum of one-year supervisory experience with the geriatric population
Responsibilities include, but are not limited to:
Supervision of LPN Supervisors, Medication Aides and Resident Care Coordinators.
Responsible for keeping the Wellness Director informed regarding residents and staff.
Liaison between resident’s medical concerns and reporting concerns to family.
Oversee medication issues with the Medication Aides weekly and is responsible to report all issues to the Director of Resident Services and Primary Care Provider.
Work independently and delegate assignments to the Wellness Staff.
Act as back-up to review Medical Evaluations (3122) prior to admission with the LPN Supervisor to ensure all orders are reconciled in Point Click Care .
Perform LPN Supervisor functions as needed.
Maintain organization in the Wellness Office, Wellness Cabinets, Nursing Station, Chart Room, Exam Rooms and Waiting Area in a professional manner. Responsible for assisting the LPNs during MD clinic visits, (i.e.
- Managing the residents and time allotted for their appointments).Assist in ongoing training staff.
Ability to physically assist a resident in need. May require lifting and bending.
On-call responsibilities
Other related duties as directed.
