Job Overview
We are looking for a Full‑Time Care Manager to join our family. The role coordinates care for high‑risk SALUD patients, particularly those in the Accountable Care Collaborative (ACC), developing comprehensive care plans that address medical and social needs.
Base Pay Range$20.00/hr – $23.00/hr

- Assist in risk stratification of high‑risk SALUD patients, emphasizing those enrolled in the ACC
- Act as a patient advocate, ensuring patients are fully informed and comfortable with visits
- Collaborate with medical, dental, behavioral health providers and other staff to deliver integrated care
- Initiate contact with patients assigned to the ACC
- Assess patients’ medical dental, behavioral, and social needs
- Develop comprehensive care programs with internal and external resource referrals
- Provide care management, follow‑up, and support to high‑risk patients, especially ACC patients
- Follow e‑health records procedures
- Assist patients with applying for and accessing assistance programs
- Offer ongoing individualized support to ACC patients
- Education: Bachelor’s degree in social work or a health‑related field preferred
- Experience: Care management, patient navigation, or social work preferred; in a community health setting or with underserved populations preferred
- Knowledge, Skills, and Abilities:
- Computer literacy, including Microsoft Office (Access, Excel, PowerPoint, Word)
- Bilingual English/Spanish required
