About This Opportunity

Accord Care is seeking a Licensed Practical Nurse (LPN) who is committed to delivering compassionate, skilled care in the home setting. This role offers the opportunity to provide one‑on‑one support that promotes client health, independence, and dignity. What You Will Do
  • Provide skilled nursing care tailored to each client's unique needs.
  • Monitor health conditions and document changes in clinical status.
  • Administer medications and perform wound care.
  • Deliver G‑tube, trach, or vent support when required.
  • Educate and guide clients and families to support ongoing health at home.
  • Communicate effectively with the care team to ensure continuity of care.
Who We Are Looking For
  • Licensed as an LPN and in good standing with the Georgia Board of Nursing.
  • Experienced in a clinical setting, with at least one year preferred.
  • Certified in CPR.
  • Skilled in EMR documentation, with strong organizational ability.
  • Confident working independently in a home environment.
  • Compassionate, client‑focused, and a strong communicator.
  • Experienced with G‑tube, trach, or vent care.
  • Licensed to drive with reliable transportation.
