Schedule:

Mon–Fri 8 AM–5 PM. Full‑time with excellent benefits and a purpose‑driven practice.

Mission‑driven, board‑certified physician passionate about senior care and value‑based medicine needed to join our client’s growing PACE program. Deliver comprehensive medical services to high‑risk seniors across Los Angeles and transform how older adults experience healthcare.

• Board‑certified in Family Medicine or Internal Medicine.
• 2+ years as a physician.
• 1+ year in Geriatric, Senior, or Long‑Term Care (LTC).
• California DEA & CDS registration.
• AHA Basic Life Support (BLS) certification.
• Valid California driver’s license with reliable personal transportation, clean driving record, and auto insurance.
• 1+ year PACE experience.

• Deliver holistic primary care to PACE participants, including examinations and chronic disease management.
• Manage an own patient panel in coordination with a complex‑care RN.
• Initiate and review orders for medications, diagnostics, and referrals.
• Provide care across the PACE center, participant homes, skilled nursing facilities, assisted living facilities, and hospitals.
• Collaborate with an interdisciplinary team (behavioral health, pharmacy, rehab).
• Participate in care planning, service determination reviews, and clinical team meetings.
• Document accurately in the EMR.
• Participate in after‑hours phone call rotations, supported by nurse triage.

Monday–Friday, 8 AM–5 PM. Flexible daily schedule with occasional travel to care settings. Rotating on‑call (remote) coverage.
