Case Manager Generalist

Volunteers of America Los Angeles seeks a Case Manager Generalist to provide comprehensive case management services to low‑acuity clients.

$22.91 - $24.12 Per Hour

• Conduct comprehensive screenings and assessments with individuals and families to collect functional, environmental, psycho‑social, financial, employment, housing, educational, and health information.
• Assess barriers, develop a detailed case plan, coordinate individualized planning to meet short‑ and long‑term needs, and connect clients to service linkages and supportive systems.
• Coordinate case conferences, consult with the service team, monitor progress, and assist clients to meet agreed‑upon goals.
• Advocate for clients before judicial, community, social service, and administrative bodies; facilitate support groups and client participation as appropriate.

• Assessment: conduct regular individual meetings, develop a detailed plan of action, set goals, and secure resources.
• Coaching and Social Support: accompany participants to activities and services.
• Coordinate Care and Case Management: connect participants to appropriate services.
• Advocacy: represent participants before other agencies and community settings.
• Capacity Building: motivate participant progress.
• Outreach and Engagement: meet participants and bring them into programs.

• Must pass fingerprint clearance, background check, and verification of employment and education.

• HS/GED. Completion of VOALA Case Management training within six months of hire.

• Minimum one‑year general experience in case management or similar social services role.
• Or Bachelor’s Degree in Social Services related field in lieu of experience.

• Bachelor’s Degree in Social Work, Social Services, public administration, public health or related field.
• One year of experience working with target population.
• HMIS navigation.
