Continuum Hospice is looking for a Registered Nurse (RN) with strong clinical knowledge and supervisory experience to lead an amazing team of clinicians as their Clinical Director.
Our RN Clinical Directors steer an interdisciplinary squad of compassionate RNs, aides, social workers, and chaplains, shaping a world of care for patients and families. Collaborating closely with the Program Director, Hospice Physician, and Community Liaisons, you will orchestrate office excellence, foster growth in patient numbers, and guarantee adherence to federal, state, ACHC, and Continuum Hospice guidelines.

- Coordinate and oversee patient services, ensuring alignment with hospice program policies.
- Provide guidance and support to clinicians, fostering continuous improvement of hospice care services.
- Conduct chart audits, ensuring compliance with regulations, certification standards, and ethical care benchmarks.
- Collaborate in orienting and training nursing & clinical staff across all proficiency levels.
- Evaluate and uplift clinical staff performance during probation and annually, providing coaching.
- Lead interdisciplinary team meetings to refine patient/family care plans and resolve challenges.
- Current, unencumbered RN license.
- Preferred:
Bachelor’s degree in nursing from a NLRN‑accredited program. - Minimum 2 years of hospice, home health or community health direct care experience within the last 5 years, and at least 1 year supervisory experience.
- Demonstrated ability to manage all aspects of terminal care, including crisis intervention, pain and symptom control, bedside nursing, and family counseling.
- Supportive, sensitive approach to staff and patient/family needs.
- Computer proficiency, including EMR experience.
- Current CPR certification.
- Valid driver’s license, vehicle insurance and reliable transportation.
$155,000 – $165,000 per year (average salary range). Compensation will be based on relevant experience and specific responsibilities. Final rate will be confirmed after the interview process.
