Speech Therapist - Home Care Per Diem

Join Emerson Health as a Speech Language Pathologist (Speech Therapist) providing home care per diem services. Reporting to the Rehabilitation Team Lead, the SLP will manage comprehensive patient care, including evaluation, diagnosis, intervention, and patient/family education, developing and implementing individualized care plans in the home setting.

Key responsibilities:

• Comprehensive patient management in a home‑care environment.
• Collaborate with multidisciplinary teams, including specialty care groups such as Parkinson’s Disease, Covid Recovery, and Post‑Acute Stroke care.
• Provide patient and family education and support to foster treatment goals.
• Participate in ongoing clinical development through education, in‑services, and care management groups.

• Education
  • Master’s Degree in Speech‑Language Pathology from an accredited institution.
• Experience
  • At least one year of acute or rehabilitation experience preferred.
• Licensure and/or Certification
  • MA SLP license. BLS required within 30 days from hire.
• Skills
  • Clinical expertise for patient population.
  • Ability to prioritize workload and respond to urgent needs.
  • Communication, assessment, and critical thinking skills.
  • Organizational, problem‑solving, and computer skills.
  • Ability to maintain confidential information.
  • Proficiency in reading, writing, and communicating in English.
