Registered Nurse; RN Home Care

Registered Nurse (RN) – Home Care – 24 hours/week ($6,000 sign‑on bonus)

Reporting to the Nursing Supervisor, Home Care, this position provides assessment, development of individualized care planning and goal setting, instruction to patient/family, and skilled nursing services to patients/families in the home setting.

• Conduct patient assessments in the home setting.
• Develop individualized care plans and set goals.
• Instruct patients and families in self‑care and treatment plans.
• Provide skilled nursing services and monitor patient status.

• Education: Associate Degree in Nursing;
  Baccalaureate degree preferred.
• Experience: Minimum two years of medical/surgical experience; knowledge of community health nursing preferred.
• Licensure/Certification: Current Massachusetts RN license; BLS healthcare provider required.
• Skills: Basic nursing skills; communication, assessment, critical thinking, organizational, and computer skills; ability to maintain confidential information; proficiency in English; excellent customer service.
