Purpose:
Provides continuous digital observation and surveillance of assigned patients. First line of action is to verbally redirect the patient from engaging in at risk behaviors. Summons the on-site clinical staff if the patient requires assistance. Knowledgeable and supportive of patient and institutional confidentiality and related hospital policies.
Responsibilities
- Demonstrates understanding and proficiency of the use of the Video Monitoring System (VMS) Solution software.
- Maintains visual observation at all times. Verbally redirects patient over digital 2-way audio device that is in patient room. Immediately summons the on-site clinical staff if the patient requires assistance.
- Demonstrates the knowledge and skill necessary to provide the appropriate care to patients based on their individualized treatment plan as delegated by a registered nurse.
- Participates in hand-off of pertinent information/behavior about assigned patients when arriving on unit and upon completion of shift assignment.
- Provides patient with explanations as necessary, but does not counsel or provide opinions. Demonstrates behaviors in line with Service Excellence.
- Seeks help or advice as soon as possible when patient appears to pose a threat to themselves or others. Participates in a collaborative identification and reporting of patient safety issues. Assures patient environment safety. Seeks assistance with removal of any potential safety hazard from room and reports findings to clinical staff.
- Demonstrates safe work practices and attitudes, follows safety rules, works to prevent unsafe conditions and behaviors, and participates in organizational and department safety programs.
- Demonstrates understanding and proficiency of other digital health platforms for remote monitoring of patients as needed
- Participates in cross training and coverage of other digital health platforms that fall within the scope of practice for a patient care monitor.
- Ensure adherence to all HQ policies and procedures
- Assist in the monitoring and evaluating of training program’s effectiveness, success and ROI periodically and report to Management
- Collaborate with Management on resolving any specific problems and tailor programs as necessary
- Maintain and Model REACH Values (Respect, Excellence, Accountability, Compassion, Honor)
- High School Graduate/G.E.D
- Minimum one (1) year experience in hospital or clinical environment (physician office, nursing home, ambulatory services, etc.) providing direct patient care.
- Excellent verbal communication skills – most communication will be done remotely using a microphone and headset
- Ability to communicate positively and effectively with patients and hospital staff, including assessing the situation and de-escalating if necessary.
- Ability to multitask effectively, prioritizing multiple situations simultaneously and addressing each appropriately.
