Responsibilities
We are looking for a Licensed Practical Nurse (LPN) to provide compassionate and skilled care to clients in a substance abuse treatment setting at Stonington Institute, part of Universal Health Services.
Under the direction of a Registered Nurse, the LPN contributes to client care by collecting, reporting and recording objective and subjective data, participating in development of a care strategy and assisting, delegating and providing directed care.
Job Duties- Reconciles medication and performs health screenings.
- Accurately notes, transcribes and implements physician’s orders, prescriptions, faxes and pharmacy instructions.
- Assists, delegates and provides directed care.
- Develops strategy of care in consultation with other nursing personnel.
- Evaluates client’s medical condition.
- Inventories current medications and updates medication reconciliation form in chart.
- Reports to staff on medical conditions of clients and medication changes.
- Ensures appropriate medications and supplies are available and orders medications from pharmacy as needed.
- Educates clients on medications, medication management, health maintenance, substance abuse and other applicable topics.
- Monitors medical aspects of treatment of persons with substance dependence and behavioral health conditions.
- Graduate of a licensed professional nurse training program.
- Licensed in the State of Connecticut as a licensed professional nurse.
- Minimum of one year experience working in the Mental Health or Substance Abuse field preferred.
