Responsibilities
Under the direction of a Registered Nurse, the Licensed Practical Nurse (LPN) contributes to the care of clients by collecting, reporting, and recording objective and subjective data accurately and promptly. The LPN participates in the development of a strategy of care in collaboration with other nursing personnel, assists with delegated duties, and provides directed care as needed.
Job Duties- Reconcile medication and perform health screenings.
- Accurately transcribe Physician’s orders and fax prescriptions to pharmacy.
- Assist, delegate, and deliver directed care.
- Develop care strategies in consultation with nursing staff.
- Evaluate client’s medical condition and inventory medications.
- Update medication reconciliation forms in the chart.
- Report on clients’ medical conditions and medication changes to staff.
- Ensure appropriate medications and supplies are available; order medications from pharmacy as needed.
- Educate clients on medications, medication management, health maintenance, substance abuse, and related topics.
- Monitors the medical aspects of treatment for individuals with substance dependence and behavioral health conditions.

- Graduate of a licensed professional nurse training program.
- Licensed in the State of Connecticut as a licensed professional nurse.
- Minimum of one year experience working in the Mental Health or Substance Abuse field preferred.
