Clifford Beers Community Care Center in Connecticut seeks a Care Coordinator to deliver family-centered, billable and non-billable services to families based on acuity and needs. Services are provided in office, in the community, and during home visits to promote engagement.
As part of an interdisciplinary team, you will help families develop plans of care, connect with providers and resources, and document work in the electronic health record while meeting program requirements.
