Location:

Connersville, IN.

Schedule:

Day Shift Monday – Thursday, 7:30 AM – 6:00 PM, including weekend/holiday rotation coverage on IP/ARU.

About the position:
The Physical Therapist evaluates each patient and provides medically prescribed treatments and education to restore, improve, maintain optimal functioning, independence, and quality of life. The PT also supervises PTAs and manages therapy services in the department.

Overview of Responsibilities
  • Evaluate each patient using applicable diagnostic, prognostic, and functional ability assessments.
  • Reevaluate patient status as appropriate for plan of care / change of condition.
  • Interpret evaluation data, history, and medical documentation to determine rehabilitation potential and benefits from intervention.
  • Identify physical, social, and psychological needs of the patient, family, and significant others.
  • Utilize patient/family/caregiver input to set functional rehabilitation goals.
  • Document goals in functional, measurable terms with stated time frames.
  • Incorporate evaluation findings to determine specific treatment plan.
  • Involve patient/family/caregiver in treatment planning.
  • Demonstrate competence in performing all procedures and treatments required in the area, including but not exclusive to those listed in the Competency Inventory.
  • Follow established plans to ensure continuity of patient care.
  • Modify patient care based on continual assessment of the patient and treatment effects, including treatments by PTA countersigned by the PT.
  • Identify and incorporate patient’s psychological, physical, and social responses to intervention.
  • Recommend equipment or supplies for home use.
  • Provide education appropriate to each patient’s level of understanding regarding evaluation, treatment, and self responsibility.
  • Instruct in home treatment program to promote independence as appropriate.
  • Collaborate with caregivers and professionals to plan discharge from rehabilitation services.
  • Initiate discharge planning at initial evaluation.
  • Communicate with referring physicians and appropriate persons through progress notes, letters, evaluations, team meetings, and direct contact.
  • Complete clear, concise, clinically useful documentation in medical record timely according to policy.
  • Provide clinical supervision to PTAs in treatment plan implementation, counter-sign PTA documentation, supervise all times services provided.
  • Provide supervision for holders of an IN temporary permit.
  • Supervise Physical Therapy and PT Assistant students on site.
  • Participate in QA&I activities.
  • Ensure patient and departmental safety through work requests and safety procedures.
  • Supervise PTAs and rehab techs in care provision.
  • Ensure new physical therapy orders are scheduled promptly and notify supervisor when needed.
  • Participate in training and observation of medical and nursing personnel, students, and other persons in physical therapy.
  • Market and promote physical therapy services as appropriate.
  • Assume responsibility for direction of physical therapy services assigned by Manager or Director.
  • Submit capital equipment recommendations.
  • Inform Manager and Director of department needs.
  • Understand significance of volunteer opportunities and participate appropriately.
Education & Experience
  • BS, MS, or DPT degree or certification in Physical Therapy from an ACTA accredited program.
  • Preference may be given to individuals with advanced clinical skill experience or expertise.
Licensure & Certification
  • Must be fully licensed in Indiana to practice as a Physical Therapist. May work with a Temporary Permit under supervision of a PT in accordance with Indiana regulations.
  • When assigned services in Ohio, PT must hold a current OH license.
  • Basic Life Support certification is required.
