Medical Records Technician role details at Planned Systems International (PSI) via Cognito LLC, a PSI joint venture, supporting IHSC at the Montgomery Processing Center in Conroe, TX. The MRT provides medical records support to ensure accurate and timely health information for detainees in ICE custody as part of the ICE Health Services Corps (IHSC).

Overview

As a civilian contractor working for Cognito, you will utilize your abilities as a Medical Records Technician to provide the safe delivery of high-quality health care. IHSC serves as the medical expert for ICE detainee health care, operating within ICE/ERO under DHS supervision. UHOSP/USPHS Commissioned Corps Officers, civil service staff, and contractors collaborate to deliver healthcare services.

Essential Functions And Responsibilities
  • Prepares, updates, and maintains a medical record for each patient ensuring accuracy of information.
  • Maintains appointment system for patients and clinical staff where applicable.
  • Tracks compliance with internally scheduled patient appointments, issuing reminder notices to IHSC staff prior to each appointment where applicable.
  • Files/scans laboratory, radiology, and other reports in appropriate sections of the electronic medical record within prescribed timelines.
  • Routes clinical reports to appropriate clinic staff within prescribed timelines.
  • Archives clinical information from the medical record within prescribed timelines in accordance with IHSC policy and procedures.
  • Reviews all documentation for completeness and routes incomplete documents to the appropriate provider for correction prior to placing/scanning in the medical records.
  • Uses multiple systems to process narrative and tabular material to prepare, update, and maintain a medical record and provide required information to appropriate medical personnel.
  • Performs record keeping functions in accordance with program policies and maintains a high level of proficiency with electronic health records.
  • Completes required competencies initially and annually, and completes initial, annual and ad hoc training as assigned.
  • Serves as a team member for analyzing protocol practices and identifying areas for improvement.
  • Maintains patient confidentiality and HIPAA/privacy compliance in all work activities.
  • Adheres to IHSC policies, procedures, directives, and accreditation standards; participates in IHSC safety, infection control, quality improvement, and related programs.
  • Attends and participates in general/medical staff meetings.
Minimum Requirements
  • Degree/

    Education:

    High School Diploma or GED equivalent
  • Certification:
    Heartsaver CPR AED certification through the American Heart Association
  • Experience:
    • Minimum 1 year of experience in a healthcare setting as a Medical Record Technician, Unit Clerk, Medical Record Clerk, or similar role involving electronic medical/health records.
    • Minimum 1 year of direct experience with proficiency in Microsoft Office programs, specifically MS Word, Excel, Outlook, and Share Point
  • Credentialing:
    Ability to pass background check and drug screen; current physical and immunizations.
  • Knowledge:
    Basic principles of electronic medical record procedures and requirements; medical terminology.
  • Skill:
    Ability to process high volumes of medical records daily, manage intake/discharge requests, and ensure records are accurate and complete.
  • Regulatory:
    Knowledge of HIPAA/Privacy Act and confidentiality of patient records and PII.
  • Other:
    Ability to work in a multi-cultural/multi-lingual environment and to use computers, scanners, and printers throughout the workday.
Physical Demands
  • Physical exertion such as standing, walking, bending, stooping, and lifting moderately heavy equipment or patients as needed.
  • Ability to respond to medical emergencies and perform CPR/emergency care as required.
