• Ensure all activities align with the vision of Elara Caring’s leadership and Hospice team.
• Assess hospice patients and families to identify physical, psychosocial, and environmental needs using documentation, clinical records, interdisciplinary team reports, and on‑site evaluations.
• Collect data on physical, psychological, social, and spiritual factors that may influence patient/family/caregiver health status.
• Communicate significant findings, problems, and changes in health condition or unsafe facility conditions to the Clinical Supervisor, physician, facility, and other personnel involved in care.
• Provide counseling, instruction, and inclusion of patient, facility, and family in following the interdisciplinary plan of care.
• Teach hospice philosophy and pain and symptom management to staff in alternate care settings.
• Offer appropriate support at time of death and perform bereavement assessment.

• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse (RN).
• At least 1 year of experience in a clinical care setting.
• Experience in a hospice or home health environment preferred.
• 50% travel required.
• Ability to sit, stand, bend, lift, and move intermittently and lift 50–100 lbs.
• Valid driver’s license and reliable transportation.

You will report to the Clinical Team Manager.
