Oak View Health & Rehabilitation in Conway, SC is a post-acute care facility with long-term, short-term, and geriatric services. We offer a stable, in-house therapy team and excellent programs for our geriatric clientele on their journey to recovery.

• Effectively screen/evaluate patients with cognitive and/or ADL physical functioning disorders and develop an appropriate plan of care following regulatory standards.
• Utilize standardized assessments and evidence-based practice to support clinical interventions.
• Provide rehabilitative, skilled, and medically necessary treatment intervention per regulatory requirements.
• Administer procedures such as ADL retraining, therapeutic activities, exercises, modalities, and use of assistive devices.
• Assess patient progress and adjust treatments accordingly in the clinical record.
• Evaluate treatment outcomes for effectiveness and modify services as needed.
• Provide consultation and counseling to patients, families, caregivers, and other service providers.
• Communicate effectively with supervisor and health team members regarding patient progress, barriers, and treatment plans.
• Provide comprehensive discharge summary of services per regulatory requirements.

Occupational Therapy license required. Open to occupational therapists at all experience levels. New graduates welcome.
