Overview

Full‑time, Monday‑Friday 8AM‑5PM. We are a leading home‑health provider seeking a Patient Services Coordinator in Cookeville, TN.

Responsibilities
  • Ensure patient assessment visits are scheduled and performed timely.
  • Assign and schedule clinicians efficiently to maximize capacity and match skill with required care.
  • Collaborate with field clinicians to complete patient visits as ordered within the care center.
  • Manage calls from patients and staff regarding scheduling.
  • Refer clinical and performance issues to the clinical manager.
  • Monitor hospitalized patients and communicate follow‑up as needed.
  • Support the clinical manager by communicating with patients, clinicians, referral sources, business development team and office personnel.
  • Run applicable reports and respond to workflow taking appropriate action.
  • Assist with internal or external patient transfer between care centers and hospice services.
  • Maintain supply closet with routine supplies, ensuring expiration dates and packaging, and liaise with field for supply needs.
  • Perform other duties as assigned.
Qualifications
  • High school diploma or GED equivalent.
Preferred
  • One year of scheduling experience in a healthcare environment.
  • One year of administrative experience in a healthcare environment.
