• Obtain a medical history and assess the patient’s physical and emotional status, level of competency, home environment, safety factors, and family/household member’s ability to assist with care.
• Advocate for the patient by providing information on patient rights, grievance procedures, reimbursement procedures, and patient/family responsibilities; involve family in developing the plan of care.
• Participate in updating the interdisciplinary care plan every two weeks, including implementing the correct interventions to attain goals.
• Supervise and direct the care given by a Hospice Aide/CNA and evaluate such care in accordance with guidelines and regulations.
• Observe signs, symptoms, and reactions to treatments and inform the attending physician and/or PCC immediately of any changes.
• Participate in Performance Improvement activities.
• Participate in on‑call rotating schedule and provide back‑up assistance as requested.
• Accurately document in the electronic medical record using any required automated device.

• Must be a Registered Nurse licensed in the state of operation.
• Experience required, with experience in Hospice or Home Health preferred.
• Sound knowledge of nursing principles, procedures, and elements of patient family teaching.
