Travel Licensed Practical Nurse, LPN PRN
Pay
: $20 / hr.
- $35 / hr. Depending on Experience and Responsibilities
The Hospice Travel Licensed Practical Nurse (LPN) is responsible for providing quality patient care and performing technical skilled care in the patient’s home. This position assists home health aides in fulfilling their responsibilities. This is a Regional Travel Nurse to travel within Central Tennessee.
Provides technical care to patients, receives assignments and reports, provides prescribed medication under supervision of Registered Nurse, in accordance with agency policies and as permitted by local and / or state regulations.
Observes, records and reports all observations of the patient’s mental and physical care provided in progress notes and submits notes daily to the Patient Care Manager.
Obtains complete report on patients to be seen prior to visits and gives complete report of care done during visits to Patient Care Manager.
Performs special procedures / treatments for which they have been instructed and supervised by an RN. (Documentation of the LPN’s ability to complete the procedure safely will be placed in his/her personnel file and signed by RN and LPN. Such procedures include IV’s, NG tubes, draw blood, but these are not all inclusive)
Notifies the Patient Care Manager and/or physician immediately of any changes in patient’s condition that warrant attention; documents changes in the clinical record.
Participates as an IDT team member.
When requested, assists home health aides in performance of their duties. Assists the physician or the RN in performing specialized procedures.
Participates in on call according to rotating schedule.
Must be willing to travel to other offices.
Qualifications:
- Must be licensed in the agency state of operation.
- Must have a complete knowledge of technical skills required in the performance of nursing procedures with at least one (1) year of nursing experience.
- Must successfully complete on site evaluations and competencies within ninety (90) days of employment and bi-annually thereafter.
- Possess an understanding of the concepts of hospice care and his/her role as a member of an interdisciplinary care team.
