Position: LPN Charge Nurse: Sign-On Bonus & Growth

Signature HealthCARE is seeking an LPN/LVN to provide direct nursing care in a 175-bed skilled nursing facility in Putnam County. You will administer medications, perform wound care, and support residents with ADLs while maintaining dignity and safety.

Supervisory duties may include directing nursing assistants and documenting care plans. The role requires an active LPN/LVN license, CPR/BLS preferred, and at least 1 year of clinical nursing experience; long-term care experience and supervisory
