Overview

Banner Home Health is a four-star home care agency and the largest skilled home care agency in Phoenix. We provide support to our therapy teams with a robust clinical support structure and strive for the best patient outcomes with support from QI specialists and telehealth resources. This position is for a Banner Home Health Physical Therapist serving the East Valley (Pinal County;

Coolidge and Florence) with visits Monday - Friday, 8am - 5pm. The role offers autonomous but supportive work, with options to work pay per visit or hourly, mileage reimbursement, and a company cell phone and tablet for documentation in our electronic medical record system (Home Care Home Base).

You will have an opportunity to treat patients with a wide variety of clinical conditions (post-op orthopedic patients, neuro rehab, cardiac issues, and more) and to work within the Banner network of hospitals and physicians.

This position will provide visits in the East Valley (Pinal County; Coolidge and Florence) scheduled Monday - Friday 8am - 5pm.

Position Summary

This position is responsible for the evaluation, planning, administration and supervision of Physical Therapy treatment in accordance with physician referral, state and national standards, regulatory agencies, and departmental policies. The clinician demonstrates the knowledge to provide treatment appropriate to the age of the patients served.

Core Functions
  • Evaluates patient’s functional abilities and limitations. Determines if intervention is needed. Establishes a plan of care and treatment goals. Completes comprehensive evaluation within the department’s time frames. Identifies and prioritizes key limiting factors. Sets functional, measurable goals in collaboration with patient and family/caregiver. Initiates discharge planning. Recommends additions to or modifications of referring orders.
  • Implements individualized treatment plan considering the patient’s physical, social, spiritual, cultural, educational and age-specific needs. Provides skilled treatment in a safe manner. Assesses ongoing effectiveness and modifies the plan of care as indicated. Involves patient and family/caregiver in treatment decisions. Provides ongoing education to physician, nurse, patient, family/caregiver, and the interdisciplinary team.
  • Documents according to professional guidelines. Documents legibly and within department timelines. Documents evidence-based, individualized treatment techniques and plan. Documentation reflects skilled intervention and progress assessment. Reassesses treatment goals as per department policies.
  • Communicates pertinent patient information to ensure follow-through of care. Shares evaluation/treatment results and patient progress with physicians, nurses, patient, family/caregiver, and interdisciplinary team. Communicates clearly and timely. Contacts physicians for routine or difficult problems to optimize outcomes. Defends treatment rationale with standard practices and current literature.
  • Exhibits adaptability in assignments related to caseload, scheduling and staffing. Takes initiative to assist in other patient care areas as needed.
  • Demonstrates appropriate supervision and delegation to achieve safe, quality, cost-effective patient care. Supervises and delegates to PT aides and students in accordance with policies and regulatory requirements. Participates in training and orientation of new personnel. Ensures competency prior to delegation of tasks.
  • Participates in department operational activities. Attends required meetings and supports process improvement. Meets productivity standards set by the department.
  • Interacts with staff at multiple levels, physicians, patients, families, and external contacts regarding patient care. Reports and clarifies information with physician offices as needed.
Minimum Qualifications
  • Graduated from an APTA-accredited school of Physical Therapy, or equivalent.
  • Current Physical Therapy license in the state of practice. BLS certification required.
  • Ability to demonstrate clinical competency, experience, training and education relevant to the job during interviews. Arizona Fingerprint Clearance Card required if employed by Banner Home Care/Hospice or Boswell Skilled Nursing Facility.
  • Proficiency with computer software used in automated patient care and documentation tools. Familiarity with standard office equipment.

  • Additional related education and/or experience preferred.
