Community Resource Coordinator

• Assist members in connecting to community and care resources in a cost-effective manner
• Support coordination of community outreach resources and promote awareness of services
• Document and maintain community resources to align with health plan standards
• Conduct non-clinical assessments to refer members to appropriate care/services
• Perform telephonic and/or in-person outreach to connect with hard-to-reach individuals and families

• High School diploma or GED
• 1 – 2 years of related experience
• Working knowledge of Social Determinants of Health (SDOH) barriers
• Strong communication and interpersonal skills
• Ability to comply with all policies and standards

• Experience in community outreach or health education
• Familiarity with care management processes
• Bilingual skills to assist diverse populations
