Transitions of Care Coordinator

Overview

Employer Industry: Healthcare Services

• Call members post-discharge to check on their well-being and ensure they have the necessary support
• Coordinate with provider offices, home health agencies, and DME providers to meet member needs
• Assist in scheduling post-discharge NP visits
• Collaborate with the TOC team to address member needs and participate in interdisciplinary huddles
• Provide operational and administrative support to the TOC clinical team

• Salary up to $26.00 hourly

• Medical Assistant Certification is a plus; past medical office experience is highly desirable
• Minimum of 2 years in a healthcare setting, managed care, home health, or community health
• Must be available to work Monday-Friday from 10am to 6:30pm EST
• Fully bilingual in English and Spanish (preferred)

• Experience in care coordination for an elderly Medicare population
• Comfortable communicating with various stakeholders including doctors, pharmacies, and caregivers
• Proficient in using electronic health records and various computer platforms, including Google Chrome and Google Sheets
