Become a key player in community healthcare with Fraser Health as a Full-Time Community Health Nurse in Tri-Cities, B.C. Deliver essential care and support to clients in their homes and communities.
In this role, you will work independently within a collaborative interprofessional team. Your responsibilities include assessments, individualized care planning, and coaching clients with chronic conditions. Key Responsibilities:
• Establish therapeutic relationships with clients through effective communication
• Provide individualized assessments and interprofessional care planning
• Develop and support comprehensive patient/client care plans
• Facilitate timely health services and resources for clients
• Maintain accurate clinical records and evaluate progress
Requirements:
• Licensed Registered Nurse in British Columbia
• One year recent clinical experience in geriatric or adult care
• Valid BC Driver's license and access to a vehicle
• Knowledge of community healthcare resources
• Ability to work independently and in a team
