Detailed Overview
In accordance with BCCNM standards of practice and the Mission and Values of Fraser Health, the Registered Nurse/Registered Psychiatric Nurse provides treatment and care management services that are inclusive and flexible for individuals with mental illness and concurrent disorders (including substance use, developmental disorders) to meet client needs and provide integration across the care continuum. Work collaboratively within an interdisciplinary team-based model, promote rapid engagement and the delivery of client and family‑centered care.
Emphasize recovery, rehabilitation, the removal of barriers to care and a culturally‑safe, trauma‑informed approach.
Responsibilities
Coordinates care for clients by collaborating with interprofessional care team, performing intake functions, conducting mental health assessments, developing and maintaining comprehensive treatment plans based on client needs and best practices, administering prescribed medications, and monitoring and assessing clients' mental state, response to medications and efficacy of treatment plan.Plans and provides direct nursing care to clients in compliance with standards of practice by reviewing client/family history, conducting physical/psychosocial assessments, assessing vital signs, developing a care plan, and collaborating with the health care team to discuss and review the plan, implementing other treatment modalities as required such as ADLs, wound care and administering medications.Works from a strengths‑based model, using motivational interviewing and engagement techniques and collaborates with clients and professionals to develop and implement individualized care plans and assist clients to access appropriate services in the community.Establishes and maintains therapeutic relationships with clients and their informal support network by engaging stakeholders in treatment planning and evaluation, providing support and follow‑up planning, promoting self‑care, encouraging informed decision making and empowering clients to improve quality of life. Teaches clients and families about management of psychiatric conditions, concurrent substance use issues, symptoms and integration into lifestyle.Provides treatment services including crisis intervention, individual counselling and education according to various theoretical models to clients and their informal support network as required.Initiates and participates in case conferences and meetings with clients, family, service providers and interprofessional team to identify client mental health issues and coordinate ongoing follow‑up care, acting as client advocate to protect clients' rights.Consults and collaborates with interprofessional team members including MH&SUS physicians, primary care providers, team coordinator, clinical nurse educators and other health care professionals to resolve issues and adjust care plans.Monitors Extended Leave under the Mental Health Act and ensures continuity of appropriate treatment.Aids client access to services such as medical appointments, hospitals and social programs by providing information and assistance with transportation and daily living tasks.Documents assessments, care plans, observations, interventions, client/family responses, outcomes and referrals according to practice standards and Fraser Health procedures.Maintains a safe environment for clients, visitors and employees by adhering to safety and emergency practices and reporting risks.Identifies own learning needs, reviews them with…
