Fraser Health seeks a dedicated Licensed Practical Nurse to support patients and families in their homes. You will provide client assessments, care planning, and interventions tailored to chronic health conditions. Key Responsibilities:
• Establish therapeutic relationships with clients
• Conduct individualized assessments and care planning
• Provide direct care and explain treatment options
• Facilitate client transitions across care settings
• Collaborate with community resources for comprehensive care
Requirements:
• Current LPN registration with BCCNM
• One year of recent related clinical experience
• Valid BC driver’s license and personal vehicle access
• Knowledge of gerontology and chronic illness care
• Effective verbal and written communication skills
