Position: Registered Community Health Nurse Fraser Health

As a Community Health Nurse, you will work independently while collaborating with an interprofessional team to manage a diverse client caseload. Key Responsibilities:

• Conduct individualized client assessments and develop care plans

• Facilitate client transitions in healthcare services

• Provide direct nursing care and interventions

• Support Community Health Workers in client care tasks

• Participate in care conferencing with healthcare professionals

Requirements:

• Current Registered Nurse registration with BCCNM

• One year clinical experience with chronic conditions

• Valid BC Driver’s license and access to a vehicle

• Strong communication and assessment skills

• Ability to manage diverse healthcare needs
