This role is ideal for a Licensed Practical Nurse who wants to impact patient healthcare positively through collaboration and direct service. Key Responsibilities:
• Screen referrals and evaluate client conditions
• Collaborate with primary care and community services
• Implement and manage client care plans
• Advocate for clients in decision-making processes
• Document clinical records and maintain standards
Requirements:
• Practicing registration as LPN with BCCNM
• One year’s experience in outpatient/community settings
• Valid BC driver's license required
• Strong skills in health promotion and counseling
• Ability to multitask in dynamic environments
