You will empower individuals to manage chronic conditions through assessments, coaching, and interprofessional collaboration. Key Responsibilities:

• Facilitate health assessments and interventions

• Work with teams to enhance care integration

• Support client transitions and follow-up

• Conduct health promotion and counseling sessions

• Maintain accurate client care documentation

Requirements:

• LPN registration with the BCCNM is mandatory

• At least one year of relevant clinical experience

• Valid BC driver’s license with access to a vehicle

• Ability to work both independently and collaboratively

• Competence in gerontology and complex care practices
