As a member of the interprofessional team, you will coordinate care and support clients in the community setting. Your work will involve comprehensive assessments, health education, and direct nursing interventions to enhance client well-being. Collaborating effectively with other healthcare providers, you will ensure smooth transitions and continuity of care.
Key Responsibilities:
• Assess and develop shared care plans for clients
• Provide support during transitions in healthcare services
• Educate clients on self-management and health promotion
• Document clinical findings and client progress accurately
• Participate in quality improvement and risk management initiatives
Requirements:
• Current BCCNM registration as a Registered Nurse
• Minimum one year experience in community health care
• Access to a personal vehicle for work-related travel
• Excellent communication and assessment skills
• Ability to work autonomously and collaboratively
