Key Responsibilities:
• Conduct thorough assessments following client referrals
• Create intervention plans for measurable outcomes
• Oversee daily living support and ongoing challenges
• Collaborate with agencies and maintain client communication
• Document progress and maintain required case notes
Requirements:
• Diploma or Certificate in Social Services desired
• At least one year of relevant client-based experience
• Familiarity with community support services in Lower Mainland
• Current Emergency First Aid Certification
• Personal vehicle for client transportation required
