Working within a permanent framework, your mission will be to assist residents in navigating their daily challenges while promoting independence and well-being. Collaborative efforts will involve liaising with clinical teams and community agencies, ensuring seamless support and care continuity for individuals within the Cottage Semi-Independent Living Program.
Key Responsibilities:
• Conduct daily visits for support and assistance
• Promote daily living skills and safety practices
• Liaise with landlords and attend to tenancy concerns
• Encourage participation in community activities
• Maintain detailed and accurate documentation
Requirements:
• Diploma in Community Social Service
• Two years of relevant experience required
• Certifications in First Aid + CPR, WHMIS
• Ministry of Justice Criminal Record Clearance
• Access to a vehicle is an asset
