Care Coordinator - 0005N
Well Life Network invites applications for the Care Coordinator position (N). The role is full-time, Monday to Friday, 9:00 am to 5:00 pm.
Position SummaryCare Coordinators collaborate with clients and their support systems, including community providers, to ensure comprehensive health and wellness support.
Essential Accountabilities- Conduct outreach activities to engage individuals with chronic medical conditions, mental health issues, or substance use disorders.
- Perform initial and ongoing assessments to identify strengths and needs.
- Prepare and revise care plans that reflect client goals, focusing on health and wellness.
- Maintain regular contact with clients (monthly or more often), providing telephonic and face‑to‑face outreach and service planning.
- Advocate for and support clients to access resources that promote wellness and reduce ER visits and hospital admissions.
- Monitor and coordinate all client care, including insurance, treatment providers, and community resources.
- Collaborate with community providers at least monthly, contributing to a multidisciplinary team for goal‑directed care planning.
- Conduct crisis intervention when needed and follow up thereafter.
- Keep detailed, timely, and accurate records in an electronic medical record (EMR).
- Complete all required monthly documentation to ensure continuity of clients’ medical insurance and accurate billing.
- Participate in weekly team meetings to provide feedback and share resources related to client needs.
- Coordinate daily office and field schedules with team members and supervisors.
- Serve as a consultant to team members on medical, psychosocial, and substance‑use issues and social service needs.
- Attend periodic trainings to enhance skills and learn best practices.
- Engage in bi‑weekly supervision to address concerns and improve skill development.
- Maintain agency vehicles, phones, and laptops when assigned.
- Follow program guidelines as outlined in the personnel manual.
- Report issues and concerns to program administration promptly.
- Maintain confidentiality at all times.
- Participate in activities of other staff in their absence or during staff shortages.
- Represent the agency at meetings, trainings, and events.

Bachelor’s degree and a minimum of two years of experience in human services. New York State driver’s license and access to a vehicle required.
