Provides medication therapy consultations and patient education. Provides verbal and written medication consultations. Education Coordinates education, training, and competency assessments. Requirements:
• BS degree and one year Experience required.
Kentucky and Indiana Pharmacist License Registration with Kentucky Board of Pharmacy (KY hospitals), within 30 days of hire date if you are a new registrant. If previously registered, you must have registration upon hire.
