Position: D/ID Crisis Director/PDS Case Manager

Description Essential Duties and Responsibilities

  • Provide training on Crisis Prevention and Intervention.
  • Coordinate and ensure that staff receives all training needed for implementation of the crisis response system.
  • Provide technical assistance, which may include training, to private SCL providers or community caregivers in the areas of crisis prevention and management.
  • Coordinate D/s line program details, such as scheduling and assistance that includes on‑call/mobile crisis responders.
  • Data collection and analysis of crisis stabilization supports.
  • Complete required Division of Developmental and Intellectual Disabilities (DD/) monthly documents.
  • Follow‑up on D/s calls and make appropriate referrals for intensive case management.
  • Collaborate with intensive case manager to provide comprehensive crisis management support for D/s.
  • Be familiar with 202B protocols and complete when required.
  • Complete assessments from new allocations.
  • Conduct face‑to‑face and phone interviews when needed.
  • To assist the D/ in any other endeavor to ensure a quality program.
  • Working with an individual and others identified by the individual as their team to develop an Individual Plan of Care (POC) budget and spending plan, and emergency backup plan.
  • Utilize a person‑centered planning approach to identify and implement all supports needed/wanted by the individual.
  • Maximum caseload of 40 PDS clients
  • Provide any needed assistance to a PDS participant with any aspect of PDS or blended services (i.e., recruiting/hiring/managing PDS providers, completing and submitting required forms.)
  • Coordinate all services.
  • Monitor a participant’s health, safety and welfare.
  • Provide a monthly face‑to‑face home visit.
  • Be available 24 hours per day, 7 days per week, including holidays.
  • Document each contact with the participant or on behalf of the participant and the progress towards outcomes/goals
  • Resolve issues related to unmet consumer needs and/or health and safety are being jeopardized or funds in the consumer’s budget are not being utilized per the POC
  • Each staff person has a legal responsibility to report any form of abuse (physical, emotional, sexual), neglect or exploitation
  • Other duties as assigned
Requirements

Qualifications: Bachelor’s Degree in Human Services (Psychology, Sociology, Social Work, Education or Health);
Complete D/s Prevention & Intervention training and other training requirements within the time frames per SCL/MPW waiver, D/ and procedures;
Submit to and pass a criminal record check and a drug screen test.

Hours: Monday through Friday 8:00 am until 4:30 pm. Evening and weekend hours may be required.

  • Valid driver’s license and no moving violations on driving record
  • Clear Background Checks
  • Valid Social Security number or valid work permit if not a US Citizen
  • TB Risk Assessment Completed within 7 days of hire
  • Negative drug test
  • Read, adhere to and promote the MR/DD Vision Statement
  • Ability to exercise good judgment, tact, diplomacy and compassion when problem solving, handling conflict or in a crisis situation
  • Ability to complete record keeping in a timely fashion
  • Ability to get along with diverse personalities and maintain effective working relationships
  • Ability to work well with people of all races, backgrounds and needs
