Registered Nurse, Population Health – Full Time – Magnolia Staffing Solutions Job Summary
The Population Health RN will be responsible for the design and implementation of the Chronic Care Management (CCM) program. Using management and nursing skills, the RN will develop a program that meets ACO regulatory and MRHC requirements, policies, and procedures. The RN will develop policies and protocols for patient interactions, assess patients’ medical histories, and develop care plans under the direction of the primary care physician.
The RN will coordinate and implement patient education, monitor and evaluate patient progress in the CCM program, assist physicians and other disciplines in arranging appropriate resources based on patients’ needs and financial abilities, and serve as a liaison to achieve tangible outcomes for patients in the CCM program.
- Graduate of an approved school of nursing.
- Maintain CEU requirements for an RN license.
Must hold a current RN license in the state of Mississippi or a current multi‑state license within the applicant’s primary state of residency.
Experience- Experience working in population health, quality improvement, or a related field preferred.
- Knowledge of quality assurance/improvement, risk management, clinical services, or patient flow systems.
- Proficiency in Microsoft Office (Word, Excel, Outlook).
- Comprehensive understanding of population health, quality‑of‑care concepts, community health, and social determinants of health.
English required; multilingual capabilities enhance the ability to perform this role. The RN must communicate effectively and professionally with staff, management, and external individuals at all levels. Excellent phone skills, verbal and written communication, interpersonal and leadership skills, data analysis, report preparation, presentation, meeting facilitation, and the ability to provide in‑service training and consultation are essential.
Reasoning AbilityThe RN should work well with other professionals and support staff in a team‑oriented environment. The candidate must be a strategic thinker, capable of gathering and analyzing data, assessing the effectiveness of healthcare systems, and collaborating with system partners to develop solutions. Accountable, self‑starter, and able to work in a fast‑paced environment, the RN must be aggressive in developing patient‑care programs within clinical policies and ACO objectives.
