Job SummaryReporting to the Client Care Coordinator, the Home Support Worker is part of a health care team that provides assistance to families or persons during times of illness, disability, or inability to properly care for their own needs. The worker supports independence and assists with activities of daily living.
Summary of Essential Job Functions- Provide personal care such as bathing, toileting, and grooming to the client
- Complete housekeeping tasks, meal preparation, and shopping
- Provide companionship and accompany the client to community events, activities, and appointments
- Follow written plan of care as approved by the Client Care Manager
- Provide specialized care such as Alzheimer or palliative care, if required
- Document and report client progress
- Promote good mental health for the client
- Develop a caring relationship with the client and the client’s family
- Provide emotional and psychological support
- Ensure a healthy and safe living environment while following agency guidelines on lifting and transfers
- Adhere to and enforce Care Givers’ Code of Conduct
- Report all occupational health and safety hazards and related issues and follow OH&S guidelines
- Minimum grade 12 high school education and experience as a home support worker OR
- Certification as a home support worker (preferred) OR
- Willingness to commence in‑house training
- Clear Criminal Record Screening Certificate and Vulnerable Sector Check obtained within 90 days (or willing to obtain one before starting employment)
- Experience working with seniors and/or physically disabled individuals
- Medical certificate indicating good health
- Negative TB skin test
- Up‑to‑date immunization record
- Current first aid certificate or willingness to obtain
- Willingness to work flexible shifts in various locations
- Excellent communication and interpersonal skills
- Willing and able to use IT systems for scheduling and client care purposes
- Ability to work as part of a team or independently
- Good decision making and problem solving skills
- Ability to lift 25 pounds
- Ability to build good relationships with the client
- Must be caring, enjoy working with others, have a good work ethic, and possess a positive attitude
